Initial Comments:
An offsite follow-up survey conducted December 27, 2021 found that Patriot Home Care, Inc. had corrected the deficiency cited under the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.The deficiency was cited as a result of an onsite relicensure survey conducted July 12, 2021.
